Youth Application

All new scouts are required to complete an application form.

Registration costs are $1.25 per month through Recharter in upcoming February.

Additionally, Boy's Life subscription are optional at $1.00 per month through Recharter.

Our Pack’s Popcorn sales fund almost all expenses which includes all three derby kits, winter camp, many other events and all patches and awards for the year.Click on application form.

Adult Application

Parent participation is essential to ensure success of the Pack.  Some of those leadership positions are listed below.

 

Positions that work with boys:

  • Cubmaster
  • Assistant Cubmaster
  • Den Leaders
  • Assistant Den Leaders

Positions that work with adults:

  • Committee Members
  • Treasurer

All adult leaders must fill out an application.  All registration and training fees are paid by Pack 3764.

Permission Slips
For any event that requires us to leave our city requires a  consent  form filled out.

Events include any of the following:
  • Day Camp
  • Winter Camp
  • Overnight Camping
  • Sporting Events
  • Field Trips

Medical Forms
A medical form needs to be filled out once a year for the following activities:
  • Overnight camping
  • Day Camp
  • District events that may require physical activity, such as Winterfest.
  • Summer Camp
Only Part A and Part B need to be filled out, unless the cub scout is going to summer camp, in which Part C needs to be filled out with a physical examination.

Council Camps
The following are events that are sponsored by the Bay Lakes Council.  These are activities that you can choose to have your scout participate.  Registration and costs for these events are the responsibility of the scout family.
